/W_v/ might be another candidate. None of my conlangs has it.

Hmm, I see that Henrik's chart at http://www.theiling.de/ipa/ lists /w/
as a voiced fricative. Shouldn't that be a voiced approximant (as the
IPA chart lists it)?
