Somewhat off topic, but here is _a_ list, not the original (which I couldn't
find). I'm sure there are at least a few more. (Accent marks omitted)

Benitez...Cortez/s...Chavez...Diaz~Diez?...Estevez...Fernandez~Hernandez
...Galindez...Gonzalez
~Gonsalvez?(Port.?)...Gutierrez...Ibañez...Jimenez...Lopez...
Marquez...Martinez...Mendez, Melendez, Menendez (all same?)
...Montez...Narvaez
...Nuñez...Ordoñez...Perez...Ramirez...Rodriguez...Ruiz...Sanchez...Suarez...V
aldes/z... Vasquez...Velasquez...Velez...Yañez

Just double-checking in my local (S.Fla. but not Miami) phone book, I find
lots of variation between final z/s-- obviously variation within Spanish, tho
some -s forms may be Brazilian.
Surely this name-forming procedure has been studied (probably long ago!).
What is interesting is how few of them are Latinate or obviously Germanic
