The abbreviation is "care of".  And the way I've used it/seen it used is
when sending a package/letter to someone who is either temporairily living
at someone else's house or sending something to a third party because the
address of the someone the item is intended for is unknown to the sender,
but assumed to be known by the reciever.

I used to work at a college in the International Admissions office.  I
frequently recieved letters and packages address to my international
students c/o me.  Their friends/parents/whoever didn't know the student's
address (maybe because they'd just arrived) but they did know my office and
knew I'd get it to their frind/child/whatever, so the c/o'd me on the
address.  Such fun tracking everyone down.
