I think I was inaccurate...I'd actually be more inclined to say "apesh*t" on its own rather than modifying "insane": "He's gone apesh*t!" as opposed to "He's gone apesh*t insane!". "Batsh*t" is usually a modifier: "He's gone batsh*t" and "He's gone batsh*t insane" are both acceptable, but the latter is preferred. "Bugf*ck" is always a modifier: "He's gone bugf*ck" isn't used, but "He's gone bugf*ck insane" is.
