Americans generally (depending on climate) sleep with two sheets, one above
and one below; on top of the sheet sits a blanket (woven) or a comforter
(quilted) depending on personal taste and temperature. So "between the sheets"
indeed. The point of the top sheet (thus called) is that one generally has
more sets of sheets than blankets or comforters, and they can be changed more
often, as they absorb sweat and other bodily fluids, leaving the blanket/comforter
fairly clean.
