Note that during 1592-1599, Sweden was in personal union with the
Rzeczpospolita. This was broken up because the King Sigismund/Zygmunt was
defeated by the rebellious forces led by Duke Charles (later Charles IX) in
the battle of Stångebro in 1598 (it took a year of legalisms to declare the
King deposed).
There's, I suppose, no particular reason that this union had to fail that
quick - in particular if the Polono-Lithuanian nobles had allowed the King
to bring along more Polish troops, which'd made a rebel victory at Stångebro
less likely. Obviously, having Sweden more or less permanently attached to
the Rzeczpospolita would drastically alter its history - to start with, Duke
Charles' son Gustavus Adolphus would never have become King.
Perhaps a Republic of the Three Crowns isn't entirely out of the question?
