I *believe* the roadmap from IE to High German is something like this, with
the first shift being Grimm's Law:

p > f > v (initially, but still /f/)
t > th > d
k > h (before vowels), ch (before consonants and final) -- I don't know of
an h > g shift
kw > hw > w > v (still written |w|)
b > p > pf
d > t > ts (written |z|)
g > k > ch (medially/finally always, initially only in Swiss German)
gw > kw > kv (written |qu|)
bh > b
dh > d
gh > g
gwh > *gw, but I don't know of any German words with |gw|!

Obviously I didn't include Verner's Law shifts.
