The New York Times is the only paper I can think of that would use "Mr.
Bush", anyone else would use "President Bush".
You don't hear Mr/Mrs/Miss much here either in normal usage, which is
something I consider unfortunate. I treasure the few times I get called
"Mr. Taylor". I hate being called by my first name by people I don't
know personally, like, say, the people at the bank.
