Word order really is an important feature to distinguish meanings, it seems. I just might add
that the variant with the indefinite pronoun comes much closer to the original sentence
as I would translate it as (Tom, the tall one, is a doctor).
For the other part: I would not mind taking a lot out of Irish as long as your language still is
original; it is no virtue of its own to construct an non-English or non-Irish or whatever
language. And there are so many possibilites to be non-English and non-Irish ;-)
