I can't imagine any language having a word for every possibility we could think
of. In any case, i thought of another measurement - truth vs falsity. So, with
all the measurements together:

benevolent vs. malicious
beneficial vs. harmful (to speaker)
beneficial vs. harmful (to patient)
truth vs. falsity

that allows 16 combinations. some new meanings come to light:

malicious + harmful (to patient) + truth: telling something embarrassing about the patient
malicious + harmful (to patient) + falsity: spreading vicious rumors

and there could additionally be another measurement:

belief of truth vs. belief of falsity

which then makes 32 possible meanings...
