"Dorothy" is the main character in the book and movie "The Wizard of
Oz".
The movie is a gay classic, and I have read many papers comparing it
to growing up gay (suddenly being lost from your family in a world that
seems more colorful and real, competition between the positive and negative
feminine, and so on and so on). Back in the 70's and early 80's, most gay
men could quote at least a dozen lines from the movie.
Also, the woman who played Dorothy, Judy Garland, was a bit of a gay
icon, and had a great many gay friends and employees/retainers. (She gained
a bit of notoriety for this.)
So, "a friend of Dorothy" became an oblique reference to a gay man.
