Three and four, actually (ex., WOR). The rules that govern it
apparently go back to 1912 or so; and are international in
scope (post independance Philippine stations, for ex., have
callsign assignments in KX-- through KZ-- or similar). They
were US territory before 1946, and so had K--- callsigns

Possibly. There are "deletions", though, like when a station
changes format and they want to get rid of an inappropriate
callsign.

I'm pretty sure they'd have callsigns as well. It's a matter
of international regulation of broadcast bands, etc.
